I hereby acknowledge that by signing this electronic User Agreement, I will safeguard the privacy of all patients and to protect the confidentiality of their health information. I understand that my User ID, and Password used to access Med-Metrix Web System is an integral aspect of protecting this Confidential Information and shall be treated as such. I shall not disclose or discuss any Confidential Information with any person, except to the extent specifically authorized or required by law. I shall not access, view, or utilize any aspect of the system other than what is required to fulfill my engagement.
